group enrolment, you have recently received a letter and reply card in the
mail. The reply card
may also be filled out
on-line, as indicated below. By completing and submitting the form, you are requesting to receive
your free optical benefit
and the certificate for the accident policy, which has already been
automatically set in place for you. Once you submit the reply
card, an AIL Representative will contact you to deliver your no-cost
benefit and explain any additional benefits that you and your family may
qualify for.
